When you come for your appointments, please bring the following:
- Driver’s License or a valid ID
- Insurance information (copay payment if required)
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, and/or any other relevant information
- List of medications (if any)
If you are new to TCIDA or have not been seen in the practice for a while, we encourage you to enroll in our Patient Portal by clicking the green button on the upper right hand corner of this page. Once you have entered your demographic information on the Pre-Register page, you will receive Login credentials for the Patient Portal within 1-2 business days. The Patient Portal allows you to enter your medical and surgical history and update information between visits. The Patient Portal also provides the opportunity for you to securely message your provider and nurse for non-emergencies and to receive lab results, responses to questions, and summaries of patient visits. You can do this after normal business hours. The information on the Patient Portal can be accessed on your smart phone or tablet by downloading the Healow app using Google Play or Apple App Store.
Individuals that use the Patient Portal to request appointments may receive preferential scheduling for non-urgent conditions.
